1.     Use the standard protocol/amendment format, which can be found on the AFRIMS Intranet IACUC site to prepare the protocol/amendment.  Please fill in all sections (Use N/A when appropriate)

 2.     Consult with Safety/Bio-safety where indicated (MAJ Stoner), Vet Med (MAJ Hinds), Your Dept Chief, Ms. Somporn (Biostatistician) and all co/associate investigators BEFORE and DURING protocol preparation

 3.     Obtain all signatures requested on the signature sheet of the protocol.  Send amendments through your Dept Chief and the Attending Veterinarian, MAJ Hinds.  If there is safety or statistical issue have the appropriate person review the amendment before submission.

 4.     To submit the protocol/amendment for IACUC review (1) Send an e-version of the protocol to IACUC Administrator via e-mail at AngwaraA@afrims.org and (2) hard copy of the protocol signature sheet with all original signatures.
